Federal contractors and subcontractors must begin using the Voluntary Self-Identification of Disability Form for all applicants and employees by July 25, 2023.
The Office of Federal Contracts Compliance Programs (OFCCP) has updated the Voluntary Self-Identification of Disability Form (CC-305) to include new language preferred for disabilities along with additional examples of disabilities. The updated form is currently available in English; however, other languages are coming soon.
The following is an overview of the changes made to the form:
- The expiration date changed from 5/31/2023 to 4/30/2026; however, the OMB number listed on the new form did not change from the previous (OMB Control No: 1250-00045).
- The OFCCP now includes specific verbiage on who at the organization should have visibility to their responses as “No one who makes hiring decisions will see it.”
- These are the updated categories of disabilities:
- “Alcohol or other substance use disorder (not currently using drugs illegally)”
- “Cancer (past or present)”
- “Disfigurement, for example, disfigurement caused by burns, wounds, accidents, or congenital disorders”
- “Intellectual or developmental disability.”
- “Mental health conditions, for example, depression, bipolar disorder, anxiety disorder, schizophrenia, PTSD”
- “Mobility impairment, benefiting from the use of a wheelchair, scooter, walker, leg brace(s) and/or other supports"
- “Neurodivergence, for example, attention-deficit/hyperactivity disorder (ADHD), autism spectrum disorder, dyslexia, dyspraxia, other learning disabilities”
- “Partial or complete paralysis (any cause)”
- “Pulmonary or respiratory conditions, for example, tuberculosis, asthma, emphysema”
- “Short stature (dwarfism)”
- “Traumatic brain injury.”
- The following categories of disabilities were removed from the form because they have been identified differently, as stated above.
- Autism
- Depression
- Psychiatric condition, for example, bipolar disorder, schizophrenia, PTSD, or major depression.
- The new form also added past history to the choices of identification:
- “Yes, I have a disability, or have had one in the past.”
- “No, I do not have a disability and have not have one in the past.”
OFCCP Form CC-305 must be implemented by all federal contractors and subcontractors in their applicant and employee systems and processes by July 25, 2023.

Contractors and subcontractors should determine the changes needed to update their applicant tracking systems (ATS) or contact their ATS provider to update the Voluntary Self-Identification of Disability Form. This updated form should replace the existing form and be used for all applicants during the recruitment process, by employees upon hire, and during the employee re-evaluation period.
